• Please bring Proof of the identity of the employee (e.g:passport/aadhar card )
• Avoid smoking and exposure to noises at least 24 hrs. prior to assessment
• Please bring a list of medications and recent medical reports if applicable
• Please bring spectacles or contact lenses if using for vision correction
• Please bring 4 passport size photos
